I'm not sure about the pouches of fat, but I do know that when I first put on the cups they pinched my armpit area painfully. My eyes would water, and I was barely getting suction. It was so bad I had to massage just that area alone for like 10 minutes afterwards and still felt it the next day. Fortunately - I'm one month into using the domes and I don't get so much as a twinge of pain! I think my body got used to it or started pulling the skin in the right direction. Point is, it hurt a LOT when I first started. I completely forgot about how it was until I read this post. It probably will go away in your 2nd or 3rd week if you pump consistently.
