The answer to that is a little bit complicated. I generally don’t do “pump and hold” anymore. If I did, I’d keep the pressure at 50mmhg. Any higher is too painful. Right now I’m doing 7/2 “pump and release”. That means I pump, hold for 7 minutes, then release all of the pressure for 2 minutes. Actually, to explain in more detail, I pump up to 5inhg (127mmhg), then immediately release the pressure down to 3inhg (76mmhg). I do not hold at 5inhg for any length of time. The second the gauge reads 5inhg, I drop the pressure. After I drop the pressure down to 3inhg, I completely stop touching the pump. Over the remainder of the 7 minutes the pressure naturally drops down to 1 or 1.5inhg (25 to 38mmhg). Obviously if you average 76 and 25mmhg, you get 50mmhg. So I think if you could average the vacuum pressure over the course of the 7 minutes, it would only be a little over 50mmhg. For me, this kind of pressure cycling seems to produce a lot more swelling than just holding at 50mmhg. I don’t know whether more swelling actually leads to more growth. It may not. Regardless, I generally choose to assume it does, mainly because I have no other way to compare the effectiveness of different methods in real time.
